Gloucestershire Hospitals launches survey to understand how to better support children moving to adult services
We have launched a survey for teenagers and young adults with a long-term health condition, who have recently moved, or are due to move (transition) from Children's to Adult services.
Karen Pudge, Divisional Director of Quality & Nursing - Women & Children at Gloucestershire Hospitals NHS Foundation Trust said,
“Our goal is to make sure you feel safe, supported, and confident about this change.
“If you are aged 14-25, either moving towards your transition to adult services or an adult who has recently made this transition, we want to hear from you. We also welcome feedback from parents and carers.”
Most young people will change services between 16 and 18, but the Trust aims to start preparing children with long term health conditions to move from Children’s to Adult services from the age of 14.
The Trust has created the survey to help understand what matters to teenagers and young adults as they move from children’s to adult services and encourages participants to be honest about any worries or challenges.
